Sample Performance Appraisal Forms
The following Performance Appraisal forms are the traditional style, where the review is conducted at the end of the review period. These forms can be used in place of the Performance Management form. The weakness of these traditional Performance Appraisal forms is the lack of emphasis of goal-setting and mutual discussion between the employee and manager. Of the first three Performance Appraisal forms listed below, The Non-Exempt form may be most appropriate because those jobs tend to be more standardized. Another option is to use the performance factor descriptions in the Performance Appraisal forms as a guide in generating performance management goals.
